Register with us as a New Patient

Practice Boundary

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We have the right to refuse to register patients if they have a violent flag against their record.

Step 1: Upload scans of photo identity and proof of address

If possible, please deliver a photo ID along with your completed registration form to our Blackbrook Site on Lisieux Way.
This can be a passport, drivers licence or ID card. You may upload your forms online.

Step 2: Download the Forms

To register with our practice, please download the forms below, complete fully then either return them via Email to [email protected], or drop them into our Blackbrook Site on Lisieux Way.

If you do not complete and return the Summary Care Record Consent form, we will assume that you are happy to opt in to this service. More information on this service can be found here.

Please note: You must complete all relevant information on the New Patient Registration form to avoid delays in your registration with the practice.

Organ Donation
