New Patients

How to Register

To register with our practice you will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Register Online

After you have completed the form you must come to the surgery with 2 forms of identification (ID).  Please allow two working days before coming in. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

Register in Person

Please bring all three of the above forms to the surgery with 2 forms of identification (ID). Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

Medical treatment is available from the date of registration.

Although you will be officially registered with one particular doctor, you may see the doctor of your choice. However, it is sensible that you normally see the same Doctor for an individual problem so that they may get to know you.

If you live within our practice area you are welcome to register with us. Please check that you live within our catchment area.
