New Patients

How to Register

To register with our practice you will need to complete a registration form which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Please note, all registrations are completed at our Blackbrook site on Lisieux Way. We cannot process registrations at our Victoria Gate site.

New Registration Form

Please allow two working days after you have returned the fully completed registration form before coming into the practice or booking an appointment. This allows us time to process your registration.

If you become unwell within those two days, please do not hesitate to contact the practice  and we will be able to assist you. Alternatively, please phone 111 or 999 in the instance of a life threatening emergency.

Please note; you do not need proof of ID, address or immigration status to register at our practice.

If you do however, wish to register for online patient access we will require proof of ID.

Register in Person

Medical treatment is available from the date of registration.

Although you will be officially registered with one particular doctor, it may be necessary to book you in with an alternative clinician. You may also request to see the doctor of your choice and we will endeavour to accommodate this request.

We do advise however, for continuity of care, you request your same Doctor for an individual problem so that they may get to know you.

Practice Boundary

If you live within our practice area you are welcome to register with us. Please check that you live within our catchment area.
